  • State employees or officials whose annual salaries surpass the threshold or who are designated policy-makers after the applicable filing due date must file within 30 days of the change in their salary or policy-maker designation. New State employees or officials who (1) have annual salaries above the threshold or are designated policy-makers, and (2) commence employment after the applicable filing due date must file within 30 days of hire.

    2022 Annual Statement of Financial Disclosure

    The Commission on Ethics and Lobbying in Government encourages all filers to submit a FDS through the FDS Online Filing System. However, individuals who submit a paper filing in lieu of an electronic filing may complete this writable PDF form.

    A filer who is not designated as a policy maker and whose annual salary exceeds the SG-24 filing rate may request an exemption from filing a FDS. Such exemptions may be requested through the FDS Online Filing System or by completing and submitting this writeable PDF form, which must be received or postmarked on or before the filing due date.

    FDS Redaction Request Form 2021-2022 Filings

    A filer may request redaction of specific information from the copy of his or her FDS that is made publicly available. Redactions may be requested through the FDS Online Filing System or by completing and submitting this writeable PDF form. The Commission on Ethics and Lobbying in Government, at its discretion, may grant the request if the information sought to be redacted has no material bearing on the discharge of the filer’s official duties. This form may also be used to request an exemption from disclosing certain information pertaining to a spouse and/or unemancipated children. Again, at its discretion, the Commission may grant the request if (1) the spouse, on his or her own behalf or on behalf of an unemancipated child, objects to the disclosure of such information, and (2) the information has no bearing on the discharge of the filer's official duties.

2021 FDS Amendment Forms for Paper Filers

NYC COIB Related Information and Forms

At its June 28, 2016 meeting, the Commission passed a resolution to streamline FDS compliance for filers who would otherwise have to file both a FDS with JCOPE and an annual disclosure report with the New York City Conflict of Interests Board (COIB).

Under the resolution, those filers may satisfy the Public Officers Law § 73-a filing requirement under by submitting the COIB filing to JCOPE along with completing and submitting a supplemental form that includes certain additional information. The original signed and dated supplemental form should be mailed to the following address:

    Guidelines to the Random Review Program

    Executive Law § 94(9)(n) authorizes the Commission to conduct a program of random reviews of Financial Disclosure Statements submitted to the Commission pursuant to Public Officers Law § 73-a, and requires that this program be carried out in a manner detailed in these guidelines.

    FDS Spousal Affidavit [2021]

    State officers and employees required to file a FDS may also be required to provide information regarding their spouses.  Filers with extenuating circumstances that prevent them from obtaining information regarding a spouse may utilize this form and submit to the Commision on Ethics and Lobbying in Government.
